Important Health Information for the 2024-25 school year

Dear Mukilteo School District Family,

Below you will find a link to an electronic form called Annual Student Health Information. This electronic form takes the place of the paper form used in the past, saving time and resources.

Health information is something school nurses are required to collect each year in order to best plan for every student's health safety. Please review and complete this form as soon as possible so your School Nurse has the most accurate and up-to-date information about your student's mental and physical health before school starts.

PLEASE NOTE:  

• In order to complete this form, you must be logged into ParentSquare. Many families find completing ParentSquare forms works best by going into ParentSquare through a browser, not their phone's ParentSquare app.
• If you wish to complete this form on paper, please contact your student's Registrar for a copy. The form is available in several languages.
